If you're asked, please complete the survey to let us know how we're doing.
We use a variety of methods to survey our patients and you may receive a text message from ‘CUH ÎÚÑ»´«Ã½â€™ or staff may ask you to fill out a comment card, a survey on an iPad or to scan a QR code.
